Description
Issues are large game updates to the City of Heroes or City of Villains game. There are smaller updates that occur between the larger "Issues", but these are usually for special events or to fix various bugs or exploits, to tweak game performance, etc.
There have been seventeen Issues so far, with one additional expansion pack having been announced.

- Issue 6 - Along Came a Spider (jointly released with City of Villains)

- Issue 11 - A Stitch in Time
- Issue 12 - Midnight Hour
- Issue 13 - Power and Responsibility
- Issue 14 - Architect (jointly released with Architect Edition)
- Issue 15 - Anniversary
- Issue 16 - Power Spectrum
- Issue 17 - Dark Mirror
While not an actual Issue, the release of the game is often referred to as "Issue 0".

Positron has stated the desire to release three Issues a year.

- Issue 1: Tuesday, June 29th, 2004 (62 days later)
- Issue 2: Thursday, September 16th, 2004 (79 days later)
- Issue 3: Tuesday, January 4, 2005 (110 days later)
- Issue 4: Wednesday, May 4, 2005 (120 days later)
- Issue 5: Wednesday, August 31, 2005 (119 days later)
- Issue 6: Thursday, October 27, 2005 (57 days later)
- Issue 7: Tuesday, June 6, 2006 (222 days later)
- Issue 8: Tuesday, November 28th, 2006 (175 days later)
- Issue 9: Tuesday, May 1st, 2007 (154 days later)
- Issue 10: Tuesday, July 24th, 2007 (84 days later)
- Issue 11: Wednesday, November 28th, 2007 (123 days later)
- Issue 12: Tuesday, May 20th, 2008 (174 days later)
- Issue 13: Tuesday, December 2nd, 2008 (196 days later)
- Issue 14: Wednesday, April 8th, 2009 (127 days later)
- Issue 15: Monday, June 29th, 2009 (82 days later)

Notes
- The names of the various issues have traditionally been thought to be chapter names of famous books. However, Positron said in an interview [1]:
- "The names of Issues are NOT chapters of novels. Never have been, never will be. The fact that the users have found novel chapter titles that match, or are similar to, our Issue names is a testament to the tenacity of our users. I won't say we haven't been amused by this, because we have, but we don't go out of our way to find a chapter title that matches our issue in theme and content. We name the issue something cool, that's it."
- Jack Emmert decided to use the name "Issues" to describe the larger updates to coincide with the comic book theme of the game.
